  2. Complete the application accurately and thoroughly.  Programs must be scheduled and seats filled on a first-qualified, first-served basis.  Please do not delay your training by submitting incomplete forms!

  3. Submit online, fax, or mail completed forms to: Fax 818-541-0832, or 
    PDC 2340 Honolulu Avenue, Montrose, CA 91020.  
    A PDC Project Specialist will review your application then call to update you on the process.
